When the specimen does not come directly from the lung and the diagnosis does not state that the primary site of the cancer is the lung, it is considered an equivocal diagnosis. Documentation dated subsequent to the report that supports that the claimant was being treated or followed for primary lung cancer will be needed to resolve an equivocal pathology report.
