We suggest that you consider submitting a copy of the attached certification – or something similar - for each of the trusts to which your firm submits claims. It is an affirmation stating that the individual recorded on the PR page and signing the release has the sole and exclusive right to do so, and that you indemnify the Trust. This document would need to be executed by a Principal at your firm.
For example, if you were to complete the affirmation specific to the G-I Trust and submit it to Verus, it would be uploaded to every claim you have filed with G-I – past, present and future, and all estate representation issues would cease to be a problem.
